MEDICAL STATEMENT AND IMMUNIZATION Medical Statement Every student must complete a "Medical History" form provided by the Division of Student Affairs. The form must be forwarded to the UWF Health Center before registration. The University of West Florida reserves the right to refuse enrollment to any applicant whose health record indicates the existence of a condition which may be harmful to members of the University community. Immunization Requirements As a prerequisite to matriculation or registration, the Florida Board of Education requires:
Acceptable documentation for both rubeola and rubella must be from a health care provider such as your physician, clinic, health department records, military immunization records or from your high school or college/university records, etc. ADDITIONAL INFORMATION AND RECOMMENDATIONS The documented date of immunization for both measles and rubella should include the day, month, and year. However, only month and year will suffice as long as the month and year show that the immunization was given at least 13 months after month of birth. It is recommended that those students who cannot provide an immunization date after extended research, or those who present questionable diagnosis of measles, be vaccinated (revaccinated) prior to the time of matriculation/registration. All immunizations should be given in accordance with the recommendations of the Advisory Committee on Immunization Practices. Exceptions to this policy may be granted in the event of valid medical contraindications or for religious reasons. Documentation verified by physician or religious leader is required. Call 857-6346 for any questions. Documentation of immunity may be presented in person or by mail to the University Health Center, University of West Florida, 11000 University Parkway, Pensacola, FL 32514. If mailing documentation, be sure it contains your name, birthdate, and social security number. Photocopies are acceptable. Originals will not be returned. ORIENTATION The Division of Student Affairs, in collaboration with Enrollment Services, the University Advising Center and various other units of the University provide orientation programs for new students and parents. Student orientation programs for freshman students entering in the Fall are scheduled throughout the summer and feature a two day agenda including academic advising, information on important policies and procedures, and community building activities and events. Freshman students attending summer orientation are required to stay overnight in the residence halls. An abbreviated orientation is offered in December and May for first year students entering in Spring and Summer terms respectively. Transfer orientation programs are offered in January, May, and throughout the summer for transfer students entering the University. All students entering the University will receive information about orientation with instruction on how to register for the appropriate program on-line. Contact the Office of Student Transition Programs at 474-2406 for more information. ACADEMIC ADVISING Each degree-seeking student is assigned to a faculty member to assist in planning academic programs; provide guidance in personal, academic, and professional development; and foster interaction among students and faculty. The University Advising Center provides advising for General Studies requirements for freshman and sophomore students. The Center also provides a system which alerts freshmen and sophomores to potential academic problems. All degree-seeking junior and senior students are assigned faculty advisors by the department housing the program in which the student is majoring. Junior and senior students should contact the chairperson of their major department to obtain the name of their academic advisor. Advising for College of Business students is available within each academic department. The Professional Studies Student Center (PSSC) is the central office of advisement for students who are majoring in Health, Leisure, and Exercise Science, Criminial Justice, Legal Studies, Teacher Education, and Social Work. In addition, PSSC is responsible for assisting transfer students in the areas of recruitment and retention. Finally, PSSC provides oversight for student programming and student events in The College of Professional Studies. Students who want to request information concerning student teaching and certification should contact the department of Teacher Education. Students in the College of Arts and Sciences should contact their major department for advising. Teacher Education Student Services (TESS) is the central campus office for all student information relating to teacher education. This office admits students into teacher education programs and into student teaching, distributes certification applications, and makes recommendations for certification. LIGHTHOUSE New students can check their admissions status through Lighthouse at lighthouse.uwf.edu. Once admitted, students can also check their financial aid status and register for classes for the first time. REGISTER FOR CLASSES Refer to information on Registration and Academic Policies. OBTAIN NAUTILUS CARD All Pensacola campus students are required to purchase a Nautilus Card. Refer to information on fees. OBTAIN PARKING DECAL Parking a vehicle on campus requires a parking decal which may be purchased at the Cashier's Office, Building 20 or online at https://nautical.uwf.edu/compass/. REVIEW STUDENT RIGHTS AND RESPONSIBILITIES Obtain a copy of the Student Life handbook for more information on topics below: Code of Student Conduct The University seeks to provide an environment which encourages the thoughtful development of intellectual, social, and moral standards. Student conduct is expected to be lawful, and students are expected to abide by all University regulations and the Code of Student Conduct, as published in the Student Life handbook. Grievance All students may bring grievances to the attention of University personnel, and they will receive prompt and fair disposition of grievances as outlined in the Student Life handbook. Prohibition of Harassment Harassment is prohibited, whether on the basis of race, color, religion, gender, national origin, age, physical disability, marital status, or veteran status. Harassment is defined as conduct which unreasonably interferes with a student's status or performance by creating an intimidating, hostile, or offensive working or education environment. Sexual harassment is defined as unwelcome or unsolicited sexual advances, requests for sexual favor, and other verbal or physical conduct of a sexual nature. Guidance, support, and assistance concerning discrimination are available from Office of Human Resources. Any student believing they have been harassed may bring complaints to the Office of Human Resources. Use of Instructional Space and Resources University facilities and equipment are intended primarily for the use of the faculty and students currently enrolled in courses of instruction. Students who have completed registration, including the payment of fees for the current semester, and whose names appear on the final class rolls, are authorized to attend classes and to use University instructional areas, facilities, equipment, and designated services. Students, including those continuing work on theses and dissertations, who are not enrolled for the current semester, are not authorized to use instructional space and resources. Students may not attend classes, including use of instructional space, facilities, designated library services, and equipment, for which they have not paid fees or from which they have withdrawn. |
