MEDICAL STATEMENT AND IMMUNIZATION
Medical Statement
Every student must complete a "Medical History" form provided by
the Division of Student Affairs. The form must be forwarded to the UWF Health
Center before registration. The University of West Florida reserves the right to
refuse enrollment to any applicant whose health record indicates the existence
of a condition which may be harmful to members of the University community.
Immunization Requirements
As a prerequisite to matriculation or registration, the Florida Board of
Education requires:

all students born
after 1956 to present documented proof of immunity to rubeola (also known
as measles, red measles, or ten-day measles) and
|
| B. |
all students less
than 40 years of age to present documented proof of immunity to rubella |
ACCEPTABLE DOCUMENTATION

RUBEOLA
Students can be considered immune to rubeola only if they have
documentation of at least one of the following:
|

1. |
Documentation of
immunization with two (2) doses of live virus vaccine on or after the
first birthday. Persons vaccinated with killed or an unknown vaccine prior
to 1968 must be revaccinated. Persons born before 1957 may be considered
to have had natural infection and therefore do not need rubeola vaccines.
The two rubeola vaccines must be at least 30 days or more apart. Or,

2. |
Laboratory
(serologic) evidence of rubeola immunity (titer). Or,
|
|
3. |
A written, dated
statement signed by a physician on his/her stationery which specifies the
date seen and states that the person has had an illness characterized by a
generalized rash lasting three (3) or more days, a fever of 101 degrees
Fahrenheit or greater, a cough and conjunctivitis, and, in the physician's
opinion, is diagnosed to have had the 10-day rubeola measles.

RUBELLA
Students can be considered immune to rubella only if they have
documentation as follows:

1. |
Documentation of
immunization with live rubella virus vaccine on or after the first
birthday. Or,
|

2. |
Laboratory
(serologic) evidence of rubella immunity (titer). |
Note: Having had the disease does not prove immunity.
For both rubeola and rubella, acceptable documentation
must be from a health care provider such as your physician, clinic, health
department records, military immunization records or from your high school or
college/university records, etc.
ADDITIONAL INFORMATION AND RECOMMENDATIONS
The documented date of immunization for both measles and
rubella should include the day, month, and year. However, only month and year
will suffice as long as the month and year show that the immunization was given
at least 13 months after month of birth.
Also, since revaccination causes no adverse medical
consequences, it is recommended that those students who cannot provide an
immunization date after extended research, or those who present questionable
diagnosis of measles, be vaccinated (revaccinated) prior to the time of
matriculation/registration. All immunizations should be given in accordance with
the recommendations of the Advisory Committee on Immunization Practices.
Exceptions to this policy may be granted in the event of
valid medical contraindications or for religious reasons.
Documentation of immunity may be presented in person or by
mail to the University Health Center, The University of West Florida, 11000
University Parkway, Pensacola, FL 32514. If mailing documentation, be sure it
contains your name, birthdate, and social security number. Photocopies are
acceptable. Originals will not be returned.
ORIENTATION
Each semester Student Affairs sponsors an orientation for new freshman and
sophomore students. Parents of new students are also encouraged to attend an
orientation session. Orientation lasts two days and is designed to acquaint
students with the University and to facilitate students' choices of courses,
registration, and completion of other necessary procedures. During orientation,
student mentors meet as peer advisors with incoming freshmen.
New students receive information about orientation by mail
after they have been admitted. The University offers several orientation
sessions for transfer students (students entering UWF with 60 or more semester
hours). Transfer orientations are held before the beginning of the Fall and
Spring Semesters. Invitations are mailed to newly admitted transfer students.

ACADEMIC ADVISING
Each degree-seeking student is assigned to a faculty member to assist in
planning academic programs; to provide guidance in personal, academic, and
professional development; and to foster interaction among students and faculty.
The University Advising Center provides advising for
General Studies requirements for freshman and sophomore students. The Center
also provides a system which alerts new freshmen and sophomores to potential
academic problems. Students are notified by letter of unacceptable progress and
urged to meet with their advisors to discuss possible remedies.
All degree seeking junior and senior students are assigned
faculty advisors by the department housing the program in which the student is
majoring. Junior and senior students should contact the chairperson of their
major department to obtain the name of their academic advisor.
Advising for College of Business students is available
within each academic department; College of Professional Studies students
majoring in teacher education should contact Teacher Education Student Services,
all other College of Professional Studies students should contact their major
department . Students in the College of Arts and Sciences should contact their
major department for advising.
Teacher Education Student Services (TESS) is the central
campus office for all student information relating to teacher education. This
office admits students into teacher education programs and into student
teaching, distributes certification applications, and makes recommendations for
certification.
LIGHTHOUSE
New students can check admissions status through Lighthouse at
lighthouse.uwf.edu. Once admitted, students can also check their financial aid
status and register for the first time.

All Pensacola campus students are required to purchase a Nautilus Card.

Parking a vehicle on campus requires a parking decal which may be purchased
at the Cashier's Office, Building 20.

Code of Student Conduct
The University seeks to provide an environment which
encourages the thoughtful development of intellectual, social, and moral
standards. Student conduct is expected to be lawful, and students are expected
to abide by all University regulations and the Code of Student Conduct, as
published in the Student Life Handbook.
Grievance
All students may bring grievances to the attention of
University personnel and they will receive prompt and fair disposition of
grievances as outlined in the Student Life Handbook.
Prohibition of Harassment
Harassment is prohibited, whether on the basis of race,
color, religion, gender, national origin, age, physical disability, marital
status, or veteran status. Harassment is defined as conduct which unreasonably
interferes with a student's status or performance by creating an intimidating,
hostile, or offensive working or education environment. Sexual harassment is
defined as unwelcome or unsolicited sexual advances, requests for sexual favor,
and other verbal or physical conduct of a sexual nature.
Guidance, support, and assistance concerning
discrimination are available from designated on-campus advocates, or the Equal
Opportunity and Diversity Office. Any student believing he or she has been
harassed may bring complaints to the Equal Opportunity Office.
Use of Instructional Space and Resources
University facilities and equipment are intended primarily
for the use of the faculty and students currently enrolled in courses of
instruction. Students who have completed registration, including the payment of
fees for the current semester, and whose names appear on the final class rolls,
are authorized to attend classes and to use University instructional areas,
facilities, equipment, and designated services. Students, including those
continuing work on theses and dissertations, who are not enrolled for the
current semester are not authorized to use instructional space and resources.
Students may not attend classes, including use of
instructional space, facilities, designated library services, and equipment,
for which they have not paid fees or from which they have withdrawn.
